- Casapinta Design Group Limited (Hereinafter referred to as “The Company”) is committed to processing information in accordance with the General Data Protection Regulation (GDPR).In order to comply with the GDPR, the Applicant needs to consent to the processing and storage of his/her personal data.
- The Applicant hereby gives his/her consent to the Company to be in possession of his/her Covid-19 swab test result, as well as the above personal identification details (Collectively referred to as “Personal Information”).
- The Applicant hereby gives his/her consent to the Company to process his/her personal information, to share such information with the relevant health authorities should it be necessary to do so.
- The Applicant shall release the Company from all liability in relation to administering and issuing a result for Covid-19. The Applicant shall release and discharge all claims, whether known or unknown, against the Company and its agents and service providers which may arise during and after the test has been administered, except only to the extent caused by the sole negligence or intentional misconduct of the Company or of its agents and service providers.
